Based on 13 years cohort study in Jakarta, the prevalence of smoking in 1988 among Indonesian male was high, up to 65.6 % and 8.9 % in female, besides 15.3 % and 0.7 % who already quitted respectively. The age of smoking habits begin at age 8 years, and the oldest 50 years. The amount of cigarettes range from 1-9 sticks up to 36 sticks per day. Kretek (Clove) was the first choice compare to regular cigarettes. The risk for cardiovascular event was 3 times significantly higher among smoker compare to non smoker, and also 3 times higher either who smoke clove or regular cigarettes.
Indonesian population in 2007 exceeds 200 million, and around 100 million are males. The estimated numbers of male age 8 years and up about 80 millions, so the numbers of people who smoke 65.6 % times 80 millions or 40 million. If we calculated from female using the same formula 8.9 % times 80 millions means 7 millions. The total of Indonesian people who smoke at least 47 millions. Data from Monica Jakarta after 8 years intervention the prevalence of smoking reduced to 38.3 % in male and 1.9% in female. If we calculated to our population the numbers of people who smoke 38.3 % plus 1.9 % times 80 millions or 32 millions . The esteemed number of cigarettes per days between 32 million to 47 million using only 1 sticks , if the average increase up to 5 sticks, the number of cigarettes per days will be 160 .000.000.000 – 235.000.000.000. The smoking burden in Indonesia not only the high prevalence of people who smoke, but also the advertisement of smoking found in every where from big cities to villages, in small gang to high ways, in all media communication included TV.
Smoking burden not only comes from cigarettes but also from cars transportation as well as forest fired which always happened every year.
Legislation on smoking free country and fresh air nations must be target for government policy in badly needed to overcome this huge burden, and impossible if only non governmental organization fight alone.
